Fall NCDE District-Wide Meeting Announcement
Our 2014 Fall NCDE District-Wide meeting will be held on Saturday, November 15 at the Garner Civitan Clubhouse. The Garner Clubhouse is located at: 1005 Poole Drive, Garner, NC, 27529. Check-in will open at 8:00 a.m. and the meeting will begin at 9:00 a.m. Registration is $18.00 per person and includes morning coffee/snack, meeting and lunch.
Our program will include a special presentation from fellow Civitan, Dr. Daniel Hall, on the results of the Civitan membership research. We will also have an update from our District 3 International Director Bob Jones ,and presentations about our district projects.
There will be two ways to register for this meeting.
Online registration deadline is November 10, 2014.
Regular mail registration deadline is earlier, November 7, 2014. (This means in Helga’s mailbox, not mailed on the 7th. If there is any doubt, please use online registration)

Individual:
Register one person at a time. If the individual would like to register a spouse or friend at the same time, this is possible through the bring a guest option. Please provide requested information for each guest.
Each registration must provide an individual email address. (If guests are added, then no additional email address is required for the guest).
Payment. Please choose one payment method from the following 3 options.
Personal credit/debit card (payment goes to NCDE).
Personal PayPal Account (payment goes to NCDE). For more information about setting up your own PayPal account, please visit: https://www.paypal.com/home
Indicate the check payment option and send check to Helga Fasciano, NCDE Convention and Meeting Chair.
Club Registration: If the club treasurer or designee is charged with sending in one registration and check for their club members, please follow these instructions:
Send in the form and club check to Helga Fasciano so that it reaches Helga’s mailbox by November 7, 2014

Online register the individuals from your club using the same procedure as indicated for the individual registration directions above. Any correspondence regarding the convention registration and updates will go to the email address provided. Each primary registration may add up to 12 guests!!

Use the registration form for regular snail mail registration available on the district website or from your club president.
Make checks payable to NCDE.
